缺血性脑血管病患者侧支循环建立的相关影响因素及与神经功能损伤程度的关系分析

摘要目的探讨缺血性脑血管病患者侧支循环建立的相关影响因素及与神经功能损伤程度的关系。方法回顾性分析2017年10月～2020年5月九江市第一人民医院收治的148例缺血性脑血管病患者的临床资料，按照数字减影血管造影（DSA）检查侧支循环建立情况分为侧支循环建立组（87例）与侧支循环未建立组（61例）。分析影响缺血性脑血管病患者侧支循环建立相关因素及与神经功能损伤程度的关系。结果侧支循环建立组的美国国立卫生院神经功能缺损评分（NIHSS）评分≤3 分、梗死体积直径≤15 mm 比例高于侧支循环未建立组，差异有统计学意义（P＜0.05）；Logistic 回归分析显示，高血压、吸烟史、糖尿病、同型半胱氨酸≥15 μmol/L是影响缺血性脑血管病患者侧支循环建立的高危因素（P＜0.05 且OR≥1）。结论侧支循环建立能够减小缺血性脑血管病患者梗死体积，减轻神经损伤程度，而缺血性脑血管病患者侧支循环建立受多种因素影响，临床针对危险因素需加强防治，以改善患者预后。

Abstract：Objective To investigate the related influencing factors of collateral circulation establishment in patients with ischemic cerebrovascular disease and its relationship with the degree of neurological impairment.Methods The clinical data of 148 patients with ischemic cerebrovascular disease admitted to Jiujiang First People′s Hospital from October 2017 to May 2020 were retrospectively analyzed.According to the collateral circulation establishment by digital subtraction angiography (DSA)，the patients were divided into the collateral circulation establishment group (87 cases)and the collateral circulation non-establishment group (61 cases).The related influence factors of the establishment of collateral circulation in patients with ischemic cerebrovascular disease and the relationship with the degree of neurological impairment were analyzed.Results The proportions of National Institutes of Health Stroke Scale (NIHSS) score ≤3 and infarct diameter ≤15 mm in the collateral circulation establishment group were higher than those in the collateral circulation non-establishment group，the differences were statistically significant (P＜0.05).Logistic regression analysis showed that hypertension，smoking history，diabetes and homocysteine ≥15 μmol/L were the high risk influence factors of the establishment of collateral circulation in patients with ischemic cerebrovascular disease (P＜0.05 and OR≥1).Conclusion The establishment of collateral circulation can reduce the infarct volume and the degree of nerve injury in patients with ischemic cerebrovascular disease，and the establishment of collateral circulation in patients with ischemic cerebrovascular disease is affected by many factors.It is necessary to strengthen the prevention and treatment of risk factors in order to improve the prognosis of patients.
