Abstract:Objective To explore the analgesic effect of Dexmedetomidine combined with Ropivacaine for transverse abdominal muscle block(TAP)after open hepatectomy.Methods A total of 50 hepatectomy patients admitted to Ganzhou People's Hospital from August 2020 to August 2021 were selected as the research subjects,and were divided into experimental group and control group according to random number table method,with 25 patients in each group.After endotracheal intubation,bilateral subcostal margin TAP block was performed in both groups under ultrasound guidance.The control group was given Ropivacaine for local anesthesia,and the experimental group was given Ropivacaine combined with Dexmedetomidine for local anesthesia.Sufentanil + Butorphanol + Tropisetron were used for patient-controlled intravenous analgesia(PCIA)before suturing.The time of first complaint of pain,time of eating and time of getting out of bed were compared between the two groups.Visual analogue scale(VAS)was used to evaluate the pain of 2,4,6 and 12 h after surgery in two groups.Heart rate(HR)and mean arterial pressure(MAP)were compared between the two groups at 30,60,120 and 180 min after surgery,and the incidence of adverse reactions within 24 h after surgery was statistically analyzed between the two groups.Results The experimental group complained of pain for the first time later than the control group,eating time and out of bed activity time earlier than the control group,the differences were statistically significant(P<0.05).VAS score of experimental group was lower than that of control group at 2,4,6,12 h after operation,the differences were statistically significant(P<0.05).HR and MAP of experimental group were lower than those of control group at 30,60,120 and 180 min after operation,the differences were statistically significant(P<0.05).The total incidence of adverse reactions 24 h after operation in the experimental group was 12.00%,lower than 32.00% in the control group,the difference was statistically significant(P<0.05).Conclusion Dexmedetomidine combined with Ropivacaine can effectively prolong the analgesic time for patients with TAP undergoing open liver resection.Dexmedetomidine can play a sedative and stable circulation effect after slowly absorbing into the blood,and can effectively reduce the occurrence of related adverse reactions,with high effectiveness and safety
