Clinical effect of Tripterygium Glycosides combined with Valsartan in the treatment of patients with stage Ⅳdiabetic nephropathy

摘要目的 探讨雷公藤多苷与缬沙坦联合治疗糖尿病肾病Ⅳ期患者的临床效果。方法 选取2017年6月～2019年6月辽宁中医药大学附属第二医院收诊的100例糖尿病肾病Ⅳ期患者作为研究对象，采用完全随机法将其分为观察组（50例）与对照组（50例）。观察组采用雷公藤多苷联合缬沙坦方法，对照组采用缬沙坦方法。比较两组患者的血肌酐、血清清蛋白、24 h 尿蛋白、白介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）、高敏C 反应蛋白（hs-CRP）水平及不良反应总发生率。结果 两组患者治疗前的血肌酐、血清清蛋白、24 h 尿蛋白、炎症因子（IL-6、TNF-α、hs-CRP）水平比较，差异无统计学意义（P＞0.05）；观察组患者治疗后的血肌酐、24 h 尿蛋白、炎症因子（IL-6、TNF-α、hs-CRP）水平均低于对照组，差异有统计学意义（P＜0.05）；观察组患者治疗后的血清清蛋白水平高于对照组，差异有统计学意义（P＜0.05）。观察组患者的不良反应总发生率（4.00%）低于对照组（18.00%），差异有统计学意义（P＜0.05）。结论 雷公藤多苷方法可有效减轻糖尿病肾病Ⅳ期患者的炎症反应，减少不良反应发生情况，值得推广使用。
Abstract：Objective To explore the clinical effect of Tripterygium Glycosides combined with Valsartan in the treatment of patients with stage Ⅳdiabetic nephropathy.Methods From June 2017 to June 2019,100 patients with stageⅣdiabetic nephropathy admitted to the Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine were selected as the research objects,and they were divided into the observation group (50 cases) and the control group (50 cases) by completely randomized method.The observation group adopted Tripterygium Glycosides combined with Valsartan method,and the control group adopted Valsartan method.The levels of serum creatinine,serum albumin,24 h urine protein,interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),high-sensitivity C-reactive protein (hs-CRP) and total incidence of adverse reactions were compared in the two groups.Results There were no significant differences in the levels of serum creatinine,serum albumin,24 h urine protein,and inflammatory factors (IL-6,TNF-α,hs-CRP) before treatment between the two groups (P＞0.05).The levels of serum creatinine,24 h urine protein and inflammatory factors (IL-6,TNF-α,hs-CRP) after treatment in the observation group were all lower than those in the control group,and the differences were statistically significant (P＜0.05).The level of serum albumin in the observation group after treatment was higher than that in the control group,and the difference was statistically significant (P＜0.05).The total incidence of adverse reactions in the observation group (4.00%) was lower than that in the control group(18.00%),and the difference was statistically significant (P＜0.05).Conclusion Tripterygium Glycosides method can effectively lighten the inflammatory response and reduce the incidence of adverse reactions in patients with stage Ⅳdiabetic nephropathy,and it is worthy of promotion and application.
