摘要目的 探讨空气压力波治疗仪在老年脑梗死下肢偏瘫患者中的应用效果。方法 选取2019年1~10月在我院神经内科接受治疗的146例脑梗死下肢偏瘫老年患者作为研究对象,按照患者的治疗意愿将其分为研究组和对照组,每组各73例。对照组患者接受常规药物治疗和康复训练,研究组患者在对照组的基础上加用空气压力波治疗仪治疗。比较两组患者治疗前及治疗后10、20、30 d 的脑卒中量表(NIHSS)、生活质量评定量表(SF-36)及下肢血流情况。结果 治疗前,两组患者的NIHSS评分、SF-36评分及下肢血流情况比较,差异无统计学意义(P>0.05)。两组患者治疗后10、20、30 d 的NIHSS评分均低于治疗前,SF-36评分均高于治疗前,差异有统计学意义(P<0.05);研究组患者治疗后10、20、30 d 的NIHSS评分均低于对照组,SF-36评分均高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后10、20、30 d 的腿围周径小于治疗前,股静脉血流速度高于治疗前,反流时间短于治疗前,差异有统计学意义(P<0.05);研究组患者治疗后10、20、30 d 的腿围周径小于对照组,股静脉血流速度高于对照组,反流时间短于对照组,差异有统计学意义(P<0.05)。结论 对老年脑梗死下肢偏瘫患者施以空气压力波治疗仪联合常规药物治疗,可有效改善患者的神经功能,在加快下肢血流速度、提升生活质量中发挥重要的作用,值得临床推广应用。
Abstract:Objective To investigate the application effect of air pressure wave therapy instrument in elderly patients with lower limbs hemiplegia and cerebral infarction.Methods A total of 146 elderly patients with lower limbs hemiplegia and cerebral infarction who treated in the department of neurology of our hospital from January to October 2019 were selected as study objects and divided into the study group and the control group according to the treatment intention of the patients,with 73 cases in each group.The control group received conventional drug treatment and rehabilitation training,while the study group received air pressure wave therapy instrument on the basis of the control group.The stroke scale (NIHSS) scores,the quality of life rating scale (SF-36) scores and lower limb blood flow before treatment and at 10,20,30 d after treatment were compared between the two groups.Results There were no statistically significant differences in NIHSS scores,SF-36 scores and lower limb blood flow in the two groups before treatment (P>0.05).The NIHSS scores of the two groups at 10,20 and 30 d after treatment were lower than those before treatment,the SF-36 scores were higher than those before treatment,and the differences were statistically significant (P<0.05).The NIHSS scores of the study group at 10,20 and 30 d after treatment were lower than those of the control group,the SF-36 scores were higher than those of the control group,and the differences were statistically significant (P<0.05).The leg circumferences of the two groups at 10,20 and 30 d after treatment were less than those before treatment,the velocities of flow in femoral vein were higher than those before treatment,the reflux times were shorter than those before treatment,and the differences were statistically significant (P<0.05).The leg circumferences of the study group at 10,20 and 30 d after treatment were less than those of the control group,the velocities of flow in femoral vein were higher than those of the control group,the reflux times were shorter than those of the control group,and the differences were statistically significant (P<0.05).Conclusion Air pressure wave therapy instrument combined with conventional drug therapy can effectively improve the nerve function of the elderly patients with lower limbs hemiplegia and cerebral infarction,and play an important role in accelerating the lower extremity blood flow speed and improving the quality of life,which is worthy of clinical promotion and application.
