Application of positive end-expiratory pressure volume test in preoperative volume assessment of patients undergoing off-pump coronary artery bypass grafting

Abstract:Objective To explore the clinical application value of positive end-expiratory pressure (PEEP) volume test in preoperative volume assessment of patients undergoing off-pump coronary artery bypass grafting (OPCABG).Methods A total of 80 cases with OPCABG treated in our hospital from May 2018 to January 2020 were selected and divided into two groups according to random number table method,40 cases in each group.The observation group underwent PEEP volume test combined with semi-horizontal passive straight leg elevation test.The control group underwent passive straight leg elevation test in semi-horizontal position only.Then the changes of invasive arterial pressure indexes were compared.The continuous cardiac output indexes by pulse profile were compared.The dim sum output at different times in the rehydration test was analyzed.Results Systolic blood pressure and diastolic blood pressure in the observation group were higher than those of the control group (P<0.05),heart rate was lower than that of the control group (P<0.05),peripheral vascular resistance index and cardiac circulation efficiency were lower than those of the control group(P<0.05),maximum pressure gradient and cardiac output level were higher than those of the control group,the differences were statistically significant (P<0.05).Conclusion For patients OPCABG,PEEP volume test combined with semi-horizontal passive straight leg elevation test can improve arterial blood pressure and cardiac function,so it can improve the volume responsiveness.
