基于护患互动达标理论的自我效能护理在妊娠糖尿病患者中的应用效果

摘要目的探讨基于护患互动达标理论的自我效能护理在妊娠糖尿病（GDM）患者中的应用效果。方法选取2017年1月至2020年12月于上饶市妇幼保健院接受治疗的80 例GDM 患者为研究对象，采用随机数字表法分为观察组和对照组，每组各40 例。对照组采用健康宣教、心理护理等常规干预方式，观察组在对照组基础上采用护患互动达标理论下的自我效能护理。比较干预前和干预2 个月后，两组患者的血清血糖[空腹血糖（FPG）、餐后2 h 血糖（2 h PG）、糖化血红蛋白（HbAlc）]水平、自我管理能力[糖尿病自我效能感（SED）量表、糖尿病自我管理行为（SDSCA）量表]评分、心理状况[症状自评（SCL-90）量表]评分。结果干预前，两组患者的FPG、2 h PG、HbAlc 水平比较，差异无统计学意义（P＞0.05）；干预后观察组的FPG、2 h PG、HbAlc 水平低于对照组，差异有统计学意义（P＜0.05）。干预前，两组患者的SDSCA、SED 评分比较，差异无统计学意义（P＞0.05）；干预后，观察组SDSCA 评分低于对照组，SED 评分高于对照组，差异有统计学意义（P＜0.05）。干预前，两组患者的心理状况比较，差异无统计学意义（P＞0.05）；干预后观察组患者的焦虑、恐惧、抑郁评分低于对照组，差异有统计学意义（P＜0.05）。结论护患互动达标理论下自我效能护理有利于帮助GDM 患者控制血糖，提高自我管理能力，调节负面情绪，值得临床推广。

Abstract：Objective To explore the application effect of self-efficacy nursing in patients with gestational diabetes mellitus(GDM)based on the theory of nurse-patient interaction reaching the standard.Methods A total of 80 patients with GDM who received treatment in Shangrao Maternal and Child Health Hospital from January 2017 to December 2020 were selected as the research objects,and were divided into observation group and control group by random number table method,with 40 cases in each group.The control group adopted routine intervention methods such as health education and psychological nursing,and the observation group adopted self-efficacy nursing under the theory of nurse-patient interaction standard on the basis of the control group.The serum glucose level(fasting plasma glucose[FPG],2 h postprandial blood glucose[2 h PG],glycosylated hemoglobin[HbAlc]),self-management ability score(selfefficacy for diabetes[SED],summary of diabetes self-care activities[SDSCA])and psychological status(symptom checklist-90[SCL-90])score of patients in the two groups were compared before intervention and 2 months after intervention.Results Before intervention,there was no significant difference in FPG,2 h PG and HbAlc levels between two groups(P＞0.05).After intervention,the levels of FPG,2 h PG and HbAlc in the observation group were lower than those in the control group,the differences were statistically significant(P＜0.05).Before intervention,there was no significant difference in SDSCA and SED scores between two groups(P＞0.05).After intervention,SDSCA score of observation group was lower than control group,SED score was higher than control group,the differences were statistically significant(P＜0.05).Before intervention,there was no significant difference in psychological status between the two groups(P＞0.05).After intervention,the scores of anxiety,fear and depression in the observation group were lower than those in the control group,the differences were statistically significant(P＜0.05).Conclusion Self-efficacy nursing under the theory of nurse-patient interaction compliance is beneficial to helping control blood glucose,improving selfmanagement ability and regulating negative emotions of patients with GDM,thus it is worthy of clinical promotion.
