基于快速康复外科理念的护理干预对鼻内镜手术患者术后疼痛及康复效果的影响

摘要目的探讨基于快速康复外科理念的护理干预对鼻内镜手术患者的影响。方法选择2020年3月至2021年3月于宜春市人民医院行鼻内镜手术的120 例患者，按照随机数字表法分为对照组和试验组，各60 例。对照组实施常规护理，试验组实施基于快速康复外科理念的护理，持续护理至患者出院。比较两组患者干预前后的心理应激、术后疼痛评分及并发症发生率、术后住院时间。结果干预前，两组患者的抑郁症筛查量表（PHQ-9）、简单自测焦虑量表（GAD-7）评分比较，差异无统计学意义（P＞0.05）；干预后，两组患者的PHQ-9、GAD-7 量表评分均低于干预前，且试验组的PHQ-9、GAD-7 量表评分低于对照组，差异有统计学意义（P＜0.05）；试验组术后6、24、72 h的视觉模拟评分法（VAS）评分均低于对照组，干预后的并发症总发生率低于对照组，术后住院时间短于对照组，差异有统计学意义（P＜0.05）。结论基于快速康复外科理念的护理干预利于减轻鼻内镜手术患者术后疼痛及负性情绪，加快术后康复。

Abstract：Objective To explore the influence of nursing intervention based on the concept of rapid rehabilitation surgery on patients undergoing endoscopic sinus surgery. Methods A total of 120 patients who underwent endoscopic sinus surgery in Yichun People′s Hospital from March 2020 to March 2021 were selected and divided into the control group and the experimental group according to random number table method, with 60 cases in each group. The control group received routine nursing, and the experimental group received nursing based on the concept of rapid rehabilitation surgery, and continued nursing until the patient was discharged. The scores of psychological stress, postoperative pain, the incidence of complications and postoperative hospital stay were compared between the two groups before and after intervention. Results Before intervention, there were no significant differences in the scores of depression screening scale of patient health questionnaire 9 (PHQ-9) and simple generalized anxiety disorder score 7 (GAD-7) between the two groups (P＞0.05). After the intervention, the scores of PHQ-9 and GAD-7 in the two groups were lower than those before the intervention, and the scores of PHQ-9 and GAD-7 in the experimental group were lower than those in the control group, the differences were statistically significant (P＜0.05). At 6, 24 h and 72 h after operation, the visual analogue scale (VAS) score of the experimental group was lower than that of the control group, the total incidence of complications after intervention of the experimental group was lower than that of the control group, and the postoperative hospital stay of the experimental group was shorter than that of the control group, the differences were statistically significant (P＜0.05). Conclusion Nursing intervention based on the concept of rapid rehabilitation surgery is helpful to reduce postoperative pain, which can reduce negative emotion and accelerate postoperative rehabilitation.
