能谱CT单能量成像与常规增强CT对门静脉血管成像图像质量的对比研究

摘要 目的 探讨能谱电子计算机断层扫描(CT)单能量成像与常规增强CT对门静脉血管成像图像质量的影响。方法 选取2020年6月至2021年6月在赣州市人民医院影像科接受CT 增强扫描的肝炎及肝硬化门脉系统异常或内镜检查证实具有食管胃底静脉曲张的100 例患者作为研究对象,采用随机数字表法分为研究组(n=50)与对照组(n=50)。研究组实施能谱CT单能量成像(GSI)增强扫描,对照组采取常规增强CT 扫描。统计两组患者一般情况、有效辐射量、容积CT 剂量指数(CTDIvol)、对比剂碘摄取量、图像质量客观与主观评分。结果 研究组有效辐射量、CTDIvol 与对照组比较,差异无统计学意义(P>0.05)。研究组对比剂碘摄取量低于对照组,差异有统计学意义(P<0.05);研究组门静脉主干、门静脉右支CT 值大于对照组,门静脉左支CT 值小于对照组,门静脉主干、门静脉左支、门静脉右支噪声低于对照组,门静脉肝内段对比度噪声比(CNR)高于对照组,差异有统计学意义(P<0.05)。门静脉肝外段CNR 及主观评分与对照组比较,差异无统计学意义(P>0.05)。结论 辐射量相当的状况下,能谱CT单能量成像在门静脉血管成像中的图像质量优于常规增强CT,且对比剂碘摄入量较少。

Abstract:Objective To investigate the effect of energy spectrum computed tomography (CT) single energy imaging and conventional enhanced CT on the image quality of portal vein angiography.Methods From June 2020 to June 2021,100 patients with hepatitis and liver cirrhosis with abnormal portal system or gastroesophageal varices confirmed by endoscopy in the imaging department of Ganzhou people′s Hospital from June 2020 to December 2021 were selected as the research objects.They were divided into study group (n=50) and control group (n=50) by random number table method.The study group performed energy spectrum CT single energy imaging (GSI) enhanced scan,and the control group took conventional enhanced CT scan.The general conditions of the two groups,effective radiation dose,volumetric CT dose index (CTDIvol),contrast medium iodine intake,objective and subjective scores of image quality were counted.Results The effective radiation dose and CTDIvol of the study group were not significantly different from the control group (P>0.05).The contrast agent iodine intake amount was lower than the control group,the difference was statistically significant (P<0.05);the CT value of the main portal vein,and the right portal vein in the study group were greater than that of the control group,the CT value of the left portal vein was lower than that of the control group,and the noise of the portal vein,the left portal vein and the right portal vein were lower than the control group,the contrast-to noise ratio (CNR) of the intrahepatic portal vein was higher than that of the control group,and the differences were statistically significant (P<0.05).Compared with the control group,the CNR of the extrahepatic portal vein and subjective score were not statistically different (P>0.05).Conclusion Under the condition of equivalent radiation dose,the image quality of energy spectrum CT single-energy imaging in portal vein vascular imaging is better than that of conventional enhanced CT,and the contrast agent iodine intake is less.
