Investigation of the status quo of depression after ischemic stroke and the application value of NIHSS score in determining the risk of post-stroke depression

Abstract:Objective To investigate the incidence of ischemic post-stroke depression (PSD) and the value of National Institutes of Health stroke scale (NIHSS) score in evaluating PSD.Methods The clinical data of 120 patients with ischemic stroke admitted to Shuyang Hospital Affiliated to Xuzhou Medical University from June 2019 to December 2020 were retrospectively analyzed.According to the occurrence of PSD,they were divided into the PSD group and the non-PSD group.The pathological data of the two groups were compared,and the relationship between the occurrence of PSD and pathological conditions was analyzed.The changes of Hamilton depression scale (HAMD) and NIHSS scores in the two groups were observed. Pearson linear correlation analysis was used to analyze the correlation between NIHSS score and HAMD factor score and total score,and the predictive value of NIHSS score on PSD was analyzed by receiver operating characteristic curve (ROC).Results Of 120 patients with ischemic stroke,42 cases (35.00%) had PSD and 78 cases (65.00%) had no PSD.The scores of anxiety/somatization,day and night change,sleep disorder,sense of despair,cognitive disorder and total score of HAMD in the PSD group were higher than those in the non-PSD group,the NIHSS score in the PSD group was higher than that in the non-PSD group,the differences were statistically significant (P<0.05). Pearson linear correlation analysis showed that NIHSS score was positively correlated with anxiety/somatization,day and night changes,sleep disorder,despair,cognitive impairment score and total HAMD score (r=5.412,4.726,4.925,5.048,5.194,6.795,all P<0.05).NIHSS score predicted the area under the curve of PSD was 0.640 (standard error=0.051, P=0.008,95%CI=0.541-0.739).When NIHSS score >7.310 points,ischemic stroke patients had a higher risk of PSD.Conclusion The incidence of PSD is higher after ischemic stroke.NIHSS score is positively correlated with HAMD factor score and total score,and has certain predictive value for PSD.
