Predictive value of diaphragmatic quantitative parameters under critical ultrasound for weaning outcomes in ICU patients undergoing mechanical ventilation

Abstract:Objective To explore the predictive value of diaphragmatic quantitative parameters under critical ultrasound for weaning outcomes in patients undergoing mechanical ventilation in intensive care unit (ICU).Methods A retrospective analysis was performed on the clinical baseline data of 82 patients undergoing mechanical ventilation in ICU of Pingxiang Mining Group Co.,Ltd.between March 2018 and May 2020.According to weaning outcomes,they were divided into weaning success group (n=63) and weaning failure group (n=19).All underwent examinations by color ultrasound diagnostic system.The diaphragmatic quantitative parameters were compared between the two groups.The predictive value of diaphragmatic quantitative parameters for weaning outcomes in ICU patients undergoing mechanical ventilation was evaluated by ROC curves.Results Ultrasonic examination showed that diaphragm excursion (DE),diaphragmatic thickness at the end of expiration (DTee),diaphragmatic thickness at the end of inspiration (DTei) and diaphragm thickening fraction (DTF) in weaning success group were significantly higher than those in weaning failure group,the differences were statistically significant (P<0.05).ROC curve analysis showed that AUC values of DE,DTee,DTei,DTF and combined test for predicting weaning failure were 0.871,0.833,0.805,0.883 and 0.979,respectively,the differences were statistically significant (P<0.05).Conclusion The predictive value of diaphragmatic quantitative parameters is high for weaning outcomes in ICU patients undergoing mechanical ventilation.
