低张胃造影MSCT动态增强扫描对胃癌术前TN分期的应用研究

摘要目的探讨低张胃造影MSCT动态增强扫描（DCE-CT）对胃癌（GC）术前TN分期的预测评估效果。方法选取2020年2月至2021年8月萍乡市人民医院收治的68 例拟行手术治疗的GC 患者作为研究对象，所有患者在术前均接受低张胃DCE-CT 检查，同时将所有患者的手术结果作为金标准。观察分析所有GC 患者的金标准结果；观察与金标准结果相比，DCE-CT 诊断GC 患者T 分期的临床效能；观察与金标准结果相比，DCE-CT 诊断GC 患者N 分期的临床效能。结果与金标准结果相比，DCE-CT 诊断GC 患者T1 分期的准确率为100.0%，灵敏性为66.7%，特异性为100.0%；诊断T2 分期的准确率为69.2%，灵敏性为90.0%，特异性为84.0%；诊断T3 分期的准确率为85.7%，灵敏性为80.0%，特异性为90.9%；诊断T4 分期的准确率为80.0%，灵敏性为66.7%，特异性为96.7%。与金标准结果相比，DCE-CT 诊断GC 患者N0 分期的准确率为72.7%，灵敏性为88.9%，特异性为88.5%；诊断N1 分期的准确率为63.6%，灵敏性为70.0%，特异性为85.2%；诊断N2 分期的准确率为75.0%，灵敏性为37.5%，特异性为96.8%；诊断N3 分期的准确率为87.5%，灵敏性为100.0%，特异性为96.3%。结论给予GC 患者实施低张胃DCE-CT 检查，能够较为准确地进行术前T 分期及N 分期预测评估，从而为临床诊疗提供更好的参考资料及依据。

Abstract：Objective To evaluate the predictive effect of dynamic enhanced MSCT dynamic enhancement scan of hypotonic gastrography (DCE-CT) on preoperative TN staging of gastric cancer (GC).Methods A total of 68 GC patients who were admitted to Pingxiang People′s Hospital from February 2020 In August,2021 for surgical treatment were selected.All patients received DCE-CT exa mination of hypozhangxia stomach before surgery,and the surgical results of all patients were used as the gold standard for comparison.The gold standard results of all GC patients were observed and analyzed.The clinical efficacy of DCE-CT in diagnosing T staging in GC patients compared with the gold standard results was observed;clinical efficacy of DCE-CT in diagnosing N staging in GC patients compared with gold standard results was observed.Results Compared with the gold standard results,the accuracy,sensitivity and specificity of DCE-CT in diagnosing T1 staging in GC patients were 100.0%,66.7% and 100.0% respectively.The accuracy,sensitivity and specificity of T2 staging were 69.2%,90.0% and 84.0% respectively.The accuracy,sensitivity and specificity of T3 staging were 85.7%,80.0% and 90.9% respectively.The accuracy,sensitivity and specificity of T4 diagnosis were 80.0%,66.7% and 96.7% respectively.Compared with the gold standard results,the accuracy,sensitivity and specificity of DCE-CT in diagnosing N0 staging in GC patients were 72.7%,88.9% and 88.5% respectively.The accuracy,sensitivity and specificity of N1 staging were 63.6%,70.0% and 85.2% respectively.The accuracy,sensitivity and specificity of N2 staging were 75.0%,37.5% and 96.8% respectively.The accuracy,sensitivity and specificity of N3 staging were 87.5%,100.0% and 96.3% respectively.Conclusion DCE-CT examination of low-tensor stomach in GC patients can accurately predict and evaluate preoperative T and N staging,so as to provide better reference and basis for clinical diagnosis and treatment.
