Differential diagnosis of troponin T,N-terminal brain natriuretic peptide and D-dimer in patients with acute myocardial infarction and early chest pain

Abstract:Objective To differentially diagnose patients with acute myocardial infarction(AMI)and early chest pain by jointly detecting the contents of whole blood troponin T(cTnT),N-terminal brain natriuretic peptide(NT-proBNP)and D-dimer(D-D)assessment of the patient′s prognosis.Methods A total of 157 patients with chest pain admitted to the emergency department in our hospital from March 2018 to October 2019 were selected as the research objects.According to the screening criteria,the patients were divided into cardiogenic chest pain group(n=88)and non-cardiogenic chest pain group(n=69),and the healthy group was used as the control group(n=46),all the subjects were tested for cTnT,NT-proBNPand D-D,and at the same time the change trend of cTnT,NT-proBNPand D-D after the patient admitted to hospital was folloued up and then the prognosis of the patients was evaluated.Results The levels of cTnT,NT-proBNPand D-D in the two patient groups were higher than those in the healthy group,the differences were statistically significant(P<0.05).The levels of cTnT and NT-proBNPin the cardiogenic chest pain group were higher than those in the non-cardiogenic chest pain group,the differences were statistically significant(P<0.05).The level of D-D in the non-cardiac chest pain group was significantly higher than that in the cardiogenic chest pain group,the difference was statistically significant(P<0.05).The receiver operator characteristic (ROC)curve analysis of its prognosis showed that the change trend of cTnT,NT-proBNP and D-D levels in whole blood could be used to evaluate the prognosis of patients with chest pain,and the sensitivity and specificity of each index were high.Conclusion The combined detection of cTnT,NT-proBNPand D-D can be used as an important indicator for the differential diagnosis of chest pain in patients with cardiogenic or non-cardiogenic chest pain,and it has important value for the prognosis of patients.
