“5A”护理模式配合目标式健康教育在慢性心力衰竭患者中的应用效果

摘要目的探讨“5A”护理模式配合目标式健康教育在慢性心力衰竭患者中的应用效果。方法选取2018年5月至2020年2月洪湖市人民医院心血管内科收治的78 例慢性心力衰竭患者为研究对象，采用时间顺序分组法分为常规组（42 例）和联合组（36 例）。常规组采用常规健康教育护理干预，联合组采用“5A”护理配合目标式健康教育的护理干预方式，比较干预结束后两组患者的自我效能感、自我护理能力、疾病知识掌握能力、生活质量、意外发生率、护理满意度及预后。结果干预后，联合组的自我效能量表（GSES）评分、自我护理能力测评量表（ESEA）评分、健康教育问卷得分高于常规组，差异有统计学意义（P＜0.05）；联合组的明尼芬达心力衰竭患者生活质量（MLHFQ）量表评分低于常规组，差异有统计学意义（P＜0.05）；联合组的意外发生率、再住院率低于常规组，护理满意度、心功能维持率高于常规组，差异有统计学意义（P＜0.05）。结论 “5A”护理模式配合目标式健康教育效果良好，可以有效提高护理质量，促进患者对疾病的认识，提高自我效能感和护理能力，改善预后，提高生活质量。

Abstract：Objective To investigate the effect of "5A" nursing mode combined with target health education in patients with chronic heart failure. Methods A total of 78 patients with chronic heart failure in the Department of Cardiovascular Medicine of Honghu People′s Hospital from May 2018 to February 2020 were selected as the research objects. They were divided into the conventional group (42 cases) and the combined group (36 cases) according to the time sequence grouping method. The conventional group was treated with routine health education and nursing intervention, the combined group adopted "5A" nursing intervention with targeted health education, compare the self-efficacy, self-care ability, disease knowledge mastery ability, quality of life, accident rate, nursing satisfaction and prognosis of patients in the two groups after the intervention. Results After intervention, the scores of general self-efficacy scale (GSES), exercise of self-care agency (ESEA) and health education questionnaire in the combined group were higher than those in the conventional group, the differences were statistically significant (P<0.05). The Minnesota living heart failure questionnaire (MLHFQ) score of the combined group was lower than that of the conventional group, the difference was statistically significant (P<0.05). The incidence of accident and the rate of rehospitalization in the combined group were lower than those in the conventional group, and the nursing satisfaction and the rate of maintaining cardiac function were higher than those in the conventional group, the differences were statistically significant (P<0.05). Conclusion "5A"nursing model combined with target-type health education has a good effect. It can effectively improve the quality of nursing, promote patients′ understanding of diseases, improve self-efficacy and nursing ability, improve prognosis, and improve the quality of life.
