Abstract:Objective To investigate the effect of laparoscopy and laparotomy on inflammatory factors levels such as tumor necrosis factor -α (TNF-α), interleukin-6 (IL-6), C-reactive protein (CRP) and cognitive function in elderly patients with colon cancer. Methods A total of 113 elderly colon cancer patients admitted to Department of General Surgery, Huiyang Sanhe Hospital of Huizhou City, Guangdong Province from April 2019 to March 2021 were selected as the subjects for prospective study. They were divided into laparoscopic group (n=57) and laparotomy treatment group(n=56) by random number table method. Patients in the laparoscopic group were treated with laparoscopy, and patients in the open treatment group were treated with traditional laparotomy. The changes of serum inflammatory factors levels before and after treatment were compared between the two groups. The cognitive function score of the mini-mental state examination (MMSE) was compared between the two groups before and 24 h after surgery. Results At 24 h after surgery, the levels of TNF-α, IL-6 and CRP in the two groups were higher than those before surgery, the differences were statistically significant (P<0.05). The levels of TNF-α, IL-6 and CRP in laparoscopic group were lower than those in laparotomy treatment group, the differences were statistically significant (P<0.05). At 24 h after surgery, MMSE scores of the two groups were lower than those before surgery, the differences were statistically significant (P<0.05).MMSE score at 24 h after operation in laparoscopic group was higher than that in laparotomy treatment group, the difference was statistically significant (P<0.05). Conclusion Compared with conventional laparotomy, laparoscopic treatment of elderly patients with colon cancer may not induce additional inflammatory responses, nor increase the risk of cognitive dysfunction, which is worthy of clinical application and promotion.
