护士分类授权管理模式在重症监护室内重症肺炎患者中的应用效果

摘要目的观察护士分类授权护理模式应用于重症监护室（ICU）内重症肺炎患者的效果。方法选取2021 年1 月至2022 年12 月中国人民解放军联勤保障部队第九〇九医院（厦门大学附属东南医院）ICU 收治的490 例重症肺炎患者作为研究对象，按照入院时间先后顺序分为对照组（240 例）和试验组（250 例）。对照组给予常规护理模式，试验组采用护士分类授权护理模式。比较两组患者干预前后血乳酸、急性生理与慢性健康状况评价（APACHEⅡ）评分、白细胞计数恢复时间、机械通气时间、退热时间及并发症发生率。结果干预后，试验组患者的乳酸水平、APACHEⅡ评分均低于对照组，乳酸清除率高于对照组，白细胞计数恢复时间、机械通气时间、退热时间均短于对照组，并发症发生率低于对照组，差异有统计学意义（P＜0.05）。结论护士分类授权护理模式可有效改善ICU 重症肺炎患者生理指标，减少并发症的发生。

Abstract：Objective To observe the effect of nurse classification authorization management model in patients with severe pneumonia in intensive care unit (ICU).Methods A total of 490 patients with severe pneumonia admitted to the ICU of the 909th Hospital of Joint Logistic Support Force(Southeast Hospital Affiliated to Xiamen University)from January 2021 to December 2022 were selected as the study objects,and they were divided into the control group (240 cases) and the experimental group (250 cases)according to the order of admission time.The control group was given routine nursing mode,and the experimental group was given nurse classification authorization management model.Blood lactic acid,acute physiological and chronic health evaluation (APACHEⅡ) score,white blood cell count recovery time,mechanical ventilation time,fever reduction time and complication rate were compared between the two groups before and after intervention.Results After intervention,the lactic acid level and APACHEⅡscore of experimental group were lower than those of control group,and the lactic acid clearance was higher than that of control group,the white blood cell count recovery time,mechanical ventilation time and fever reduction time of experimental group were shorter than those of control group,the complication rate of experimental group was lower than that of control group,and the differences were statistically significant (P＜0.05).Conclusion Nurse classification authorization management model can effectively improve the physiological indexes of ICU patients with severe pneumonia and reduce the occurrence of complications.
