Clinical application of multi-slice spiral CT three-dimensional reconstruction in the preoperative evaluation of total hip replacement for adult Crowe Ⅲhip dysplasia

Abstract:Objective To explore the clinical value of multi-slice spiral CT three-dimensional reconstruction in the preoperative total hip replacement for adult Crowe Ⅲhip dysplasia.Methods A total of 40 patients(40 hips)with Crowe Ⅲhip dysplasia admitted to the Fourth Department of Orthopaedics,Ganzhou Hospital of Traditional Chinese Medicine from September 2021 to December 2022 were selected as the study subjects,they were divided into the experimental group and the control group according to the computer generated random number method,with 20 cases in each group.The experimental group used multi-slice spiral CT three-dimensional reconstruction method preoperative,the control group underwent digital radiography (DR) anteroposterior and lateral photography preoperative.The detection of preoperative lesions,determination of true acetabulum,compliance of prostheses,surgical time,intraoperative bleeding and changes in length of both lower limbs preoperative and postoperative were compared between the two groups of patients.Results The detection rate of femoral head necrosis and joint capsule hydrops,the determination rate of true socket and the anastomosis rate of prosthesis in the experimental group were higher than those in the control group,the differences were statistically significant (P<0.05).The surgical time of the experimental group patients was shorter than that of the control group,the intraoperative blood output was less than that of the control group,the differences were statistically significant (P<0.05).There was no statistically significant difference in the length of the lower limbs between the two groups of patients preoperative (P >0.05).Postoperative,the difference in length between the two groups of patients' lower limbs was smaller than that preoperative,the difference in length of both lower limbs in the experimental group was smaller than that in the control group,the differences were statistically significant(P<0.05).Conclusion The preoperative multi-slice spiral CT three-dimensional reconstruction can fully evaluate the preoperative aseptic necrosis of the femoral head and the joint cavity effusion,can reconstruct three-dimensional images to improve the determination rate of the true acetabulum,which provides sufficient basis for the preoperative formulation of acetabular reconstruction methods,reasonable selection of the size and model of the prosthesis,has important clinical significance in shortening the operation time,reducing intraoperative bleeding and reducing the length difference between the lower limbs postoperative.
