不同缝合方法在SandersⅢ型跟骨骨折患者术中的应用比较

摘要 目的 比较三层紧密缝合法与Allgower-Donati 缝合法在Sanders Ⅲ型跟骨骨折患者术中的应用价值。 方法 选取2017年6月至2020年6月东南大学附属中大医院溧水分院的83 例Sanders Ⅲ型跟骨骨折患者为研究对象,根据手术缝合方法的不同分为观察组(43 例)与对照组(40 例)。 观察组采用三层紧密缝合法,对照组采用Allgower-Donati 缝合法。比较两组的手术疗效、手术相关指标、术后恢复情况及术后切口并发症总发生率,观察两组患者术后的Maryland 评分、视觉模拟评分法(VAS)评分及切口皮肤温度变化情况。结果 两组的手术优良率比较,差异无统计学意义(P>0.05);观察组术后1 个月的Maryland 评分高于对照组,术后3 d 的VAS 评分低于对照组,差异有统计学意义(P<0.05);两组的手术时间及术后引流时间比较,差异无统计学意义(P>0.05);观察组的皮肤切口缝合时间长于对照组,切口周围皮肤炎症反应面积小于对照组,切口愈合时间短于对照组,差异有统计学意义(P<0.05);两组的骨折愈合时间比较,差异无统计学意义(P>0.05);观察组术后2、3 d 的切口皮肤温度高于对照组,差异有统计学意义(P<0.05);两组的术后切口并发症总发生率比较,差异无统计学意义(P>0.05)。 结论 相校于Allgower-Donati 缝合法,在Sanders Ⅲ型跟骨骨折术中采用三层紧密缝合法进行缝合可促进切口愈合,减轻术后疼痛,缩小皮肤炎症反应面积。

Abstract:Objective To compare the application value of three-layer tight suture method and Allgower-Donati suture method in patients with Sanders Ⅲtype calcaneal fracture. Methods A total of 83 patients with Sanders type Ⅲcalcaneal fractures from June 2017 to June 2020 in Lishui Branch Hospital of Zhongda Hospital Affiliated to Southeast University were selected as the research objects. According to the different surgical suture methods, they were divided into the observation group of 43 cases and the control group of 40 cases. The observation group was given three-layer tight suture method, and the control group was given Allgower-Donati suture method. The surgical efficacy, surgical related indicators, postoperative recovery and the total incidence of postoperative incision complications were compared between the two groups. The postoperative Maryland score, visual analog scale (VAS) score and changes in incision skin temperature were observed between the two groups. Results There was no statistically significant difference in the excellent and good rate between the two groups (P>0.05). The Maryland score of the observation group at 1 month after surgery of the observation group was higher than that of the control group, and the VAS score at 3 days after surgery of the observation group was lower than that of the control group, the differences were statistically significant (P<0.05).There were no significant differences in the operation time and postoperative drainage time between the two groups (P>0.05). The skin incision suture time of the observation group was longer than that of the control group, and the area of skin inflammation around the incision of the observation group was smaller than that of the control group, and the healing time of incision of the observation group was shorter than that of the control group, and the differences were statistically significant (P<0.05). There was no statistically significant difference in the fracture healing time between the two groups (P>0.05). The incision skin temperature of the observation group at 2 and 3 days after surgery were higher than those of the control group, the differences were statistically significant (P<0.05). There was no statistically significant difference in the total incidence of postoperative incision complications between the two groups (P >0.05). Conclusion Compared with the Allgower-Donati suture method, the three-layer tight suture method in Sanders type Ⅲcalcaneal fracture operation can promote the healing of the incision, reduce postoperative pain, and reduce the area of skin inflammation.
