基于聚焦解决模式的护理干预在卒中后疲劳患者中的应用效果

摘要目的探讨基于聚焦解决模式的护理干预在卒中后疲劳患者中的应用效果。方法选取2020年11月至2021年5月滨州医学院附属医院治疗的疲劳严重程度量表评分≥4 的脑卒中患者120 例，采用随机数字表法分为对照组和观察组，每组各60 例。对照组实施神经内科常规护理和出院后随访，观察组在对照组基础上运用基于聚焦解决模式的护理干预，比较干预前后两组患者疲劳严重程度、神经功能缺损情况、残疾程度和社会功能以及自我效能。结果干预后，观察组疲劳严重程度量表、美国国立卫生院卒中量表、Rankin 修订量表评分低于对照组，差异均有统计学意义（P＜0.05）；干预后，观察组中文版脑卒中康复自我效能量表、Barthel 指数各维度评分高于对照组，差异均有统计学意义（P＜0.05）。结论基于聚焦解决模式的护理干预能降低卒中后疲劳患者的疲劳严重程度，促进神经功能恢复，加快康复进程，提升患者的自我效能和生活自理能力，值得临床推广。

Abstract：Objective To investigate the application effect of nursing intervention based on the solution-focused approach on post-stroke fatigue patients.Methods A total of 120 stroke patients with fatigue severity scale score ≥4 who were treated in Binzhou Medical University Hospital from November 2020 to May 2021 were selected as the research objects.According to the random number table method,the patients were divided into control group and observation group,60 cases in each group.Patients in the control group were given routine neurological nursing and followup after discharge,while patients in the observation group were given solution-focused approach on the basis of the control group.The fatigue severity,neurological deficits,the degree of disability and social functioning,the self-efficacy,the ability of daily living were compared between the two groups before and after intervention.Results After intervention,the scores of fatigue severity scale,National Institutes of Health Stroke Scale,and modified Rankin scale in the observation group were lower than those in the control group,and the differences were statistically significant (P＜0.05).After intervention,the scores of stroke serf-efficacy questionnaire and Barthel index in the observation group were higher than those in the control group,and the differences were statistically significant (P＜0.05).Conclusion Nursing intervention based on the solution-focused approach can reduce the severity of fatigue in post-stroke fatigue patients,promote the recovery of neurological function,speed up the rehabilitation process,and improve the patients′self-efficacy and self-care ability,which is worthy of clinical promotion.
