Abstract:Objective To investigate the clinical effect of lung rehabilitation combined with western medicine in the treatment of chronic obstructive pulmonary disease (COPD)complicated with pulmonary heart disease (PHD)and its effect on pulmonary function and brain natriuretic peptide.Methods A total of 120 patients with COPD and PHD admitted to Yichun People's Hospital from January 2019 to September 2021 were selected as the subjects of this study.They were divided into two groups according to the random number table,with 60 cases in each group.The control group received routine western medicine treatment,and the observation group received traditional Chinese medicine lung rehabilitation combined with routine western medicine treatment.The changes of pulmonary function index,pulmonary artery systolic pressure (PASP),total ejection isovolume (Tei)index,exercise endurance,blood gas analysis index,biochemical index and quality of life were compared between the two groups before and after treatment.Results The results of vital capacity (VC),forced vital capacity (FVC),forced expiratory volume of 1s as a percentage of the estimate (FEV1%)and forced expiratory volume of 1s (FEV1/FVC) were not significantly different between the two groups before intervention (P >0.05).After intervention,the results of VC,FVC,FFEV1%and FEV1/FVC in the two groups were higher than those before intervention,and the differences were statistically significant (P <0.05).After intervention,the results of VC,FVC,FEV1% and FEV1/FVC in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in PASP,Tei index,6 min walking test(6MWT)and St.George's respiratory questionnaire(SGRQ)score between the two groups before intervention(P>0.05).After intervention,PASP,Tei index and SGRQ score of the two groups were lower than those before intervention,and 6MWT was higher than that before intervention,and the differences were statistically significant(P<0.05).After intervention,PASP,Tei index and SGRQ score of the observation group were lower than those of the control group,and 6MWT was higher than that of the control group,and the differences were statistically significant (P<0.05).There were no significant differences in partial pressure of oxygen(PaO2) and arterial partial pressure of oxygen (PaCO2) between the two groups before intervention (P >0.05).After intervention,the PaO2 of the two groups was higher than that before intervention,and the PaCO2 was lower than that of the same group before intervention,and the differences were statistically significant(P<0.05).After intervention,the PaO2 of the observation group was higher than that of the group,the PaCO2 was lower than that of the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the results of brain natriuretic peptide(BNP),cardiac troponin (CTnI),D-dimer (D-D) and hematocrit (HCT) between the two groups before intervention (P>0.05).The results of BNP,cTnI,D-D and HCT in the two groups after intervention were lower than those in the same group before intervention,and the differences were statistically significant (P<0.05).After intervention,the results of BNP,cTnI,D-D and HCT in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine lung rehabilitation combined with Western medicine in the treatment of COPD complicated with PHD has a significant effect,which can effectively improve the cardiopulmonary function of patients,and is worthy of clinical recommendation.
