Exploration of the intervention mode of medical social workers in hypertension management

Objective To explore a new mode of intervention of medical social workers in hypertension management and provide reference for chronic disease management.Methods A total of 180 cases of community hypertension patients in Luzhou from July to August 2021 were selected as the research objects.A cross-sectional survey was conducted using typical sampling method,and questionnaires were used to evaluate their self-management ability and demand for medical social workers.Results The scores of the three dimensions of self-management in community hypertension patients in Luzhou were cognition(15.15±3.93),belief(13.83±2.55)and behavior(30.44±4.40),indicating that the selfmanagement ability was at the medium level.Community hypertension patients scored(22.75±6.31)in terms of personal knowledge and skill guidance needs,and(17.92±3.81)in terms of community service support needs,indicating that they generally had different service needs for medical social workers.Conclusion The self-management of community hypertension patients is affected by many factors,and there is a need for medical social workers.The intervention of medical social workers in hypertension management can improve patients′self-management ability through personalized services,thus improving the status quo of hypertension management.
