Risk factors of nonproliferative diabetic retinopathy in patients with hypertensive and type 2 diabetes

Abstract:Objective To explore the risk factors of nonproliferative diabetic retinopathy (DR) in hypertensive patients with type 2 diabetes. Methods A total of 100 patients with hypertension and type 2 diabetes hospitalized in the Department of Endocrine and Metabolism, the Third Hospital of Nanchang from April 2019 to December 2020 were selected as the research subjects. According to the occurrence of nonproliferative DR, they were divided into observation group (n=35, with nonproliferative DR) and control group (n=65, without nonproliferative DR). All patients were followed up for 12 months to compare the incidence of fundus related diseases between the two groups. Logistic regression was used to analyze the risk factors of nonproliferative DR in patients with hypertension and type 2 diabetes. Results The incidence of fundus related diseases in the observation group was 17.14%, which was higher than 4.62% in the control group, the difference was statistically significant (P<0.05). There were significant differences in smoking, systolic blood pressure (SBP), diastolic blood pressure (DBP), hemoglobin, total cholesterol (TC) and glycosylated hemoglobin (HbA1c) between two group (P<0.05). Multivariate logistic regression analysis showed that smoking (β=1.495, OR= 6.791, 95%CI=6.312-7.326), SBP too high (β=1.213, OR=5.457, 95%CI=4.698-7.132), DBP too high (β=1.336, OR= 6.632, 95%CI=6.035-7.214), hemoglobin increased (β=1.783, OR=4.313, 95%CI=3.235-7.491), TC increased(β=1.943, OR=5.412, 95%CI=4.951-5.838), HbAlc increased (β=1.669, OR=6.323, 95%CI=4.346-7.982) were risk factors for nonproliferative DR in hypertensive patients with type 2 diabetes. Conclusion The risk factors of nonproliferative DR in hypertensive patients with type 2 diabetes are complex, mainly including smoking, SBP, DBP, hemoglobin, TC and HbA1c. More reasonable preventive intervention measures should be taken for patients with combined risk factors to avoid the occurrence of nonproliferative DR.
