COOK球囊引产在足月妊娠高血压产妇中的应用效果

摘要 目的 探讨COOK球囊引产在足月妊娠高血压产妇中的应用效果。方法 选取2020年2月至12月惠阳三和医院产科收治的136例足月妊娠高血压产妇作为研究对象,采用随机数字表法分为对照组(68例)与研究组(68例)。对照组口服米索前列醇引产,研究组产妇利用COOK双球囊引产。比较两组产妇的分娩结局、宫颈成熟情况、产程时间、分娩方式与新生儿结局情况。结果 研究组的第一产程、总产程短于对照组,差异有统计学意义(P<0.05);两组产妇的第二产程比较,差异无统计学意义(P>0.05)。两组产妇干预前的宫颈成熟Bishop评分比较,差异无统计学意义(P>0.05);干预后研究组的Bishop评分高于对照组,差异有统计学意义(P<0.05)。两组新生儿的体重、5 min Apgar评分、新生儿窒息、高胆红素血症、新生儿感染等指标比较,差异无统计学意义(P>0.05)。研究组的产妇阴道分娩率为75.00%,高于对照组的52.94%,差异有统计学意义(P<0.05)。研究组产妇的宫缩过强、产后尿潴留、产后出血等发生率低于对照组,差异有统计学意义(P<0.05);而两组产妇的急产、羊水污染发生率比较,差异无统计学意义(P>0.05)。结论 与米索前列醇引产比较,对足月妊娠高血压产妇采取COOK双球囊引产有助于降低产妇分娩后的并发症发生率,促进宫颈成熟,缩短产妇产程,提高阴道分娩率,值得推广。

Abstract:Objective To explore the application effect of COOK balloon induced labor in full-term pregnancy women with hypertension. Methods A total of 136 full-term pregnancy women with hypertension treated in the Department of Obstetrics of Huiyang Sanhe Hospital from February to December 2020 were selected as the research objects. They were divided into the control group (68 cases) and the study group (68 cases) according to random number table method. The control group was induced by oral Misoprostol, and the study group was induced by COOK double balloon. The delivery outcomes, cervical maturity, duration of labor, the mode of delivery and neonatal outcome were compared between the two groups. Results The first stage of labor and the total stage of labor in the study group were lower than those in the control group, the differences were statistically significant (P<0.05). There was no significant difference in the second stage of labor between the two groups (P>0.05). There was no significant difference in the Bishop score of cervical maturity between the two groups before intervention (P>0.05). After intervention, the Bishop score of the study group was higher than that of the control group, the difference was statistically significant (P<0.05). There were no significant differences in neonatal weight, 5-minute Apgar score, neonatal asphyxia, hyperbilirubinemia and neonatal infection between the two groups (P>0.05). The vaginal delivery rate in the study group was 75.00%, which was higher than that in the control group (52.94%), the difference was statistically significant (P<0.05). The incidence rates of uterine contraction, postpartum urinary retention and postpartum hemorrhage in the study group were lower than those in the control group, the differences were statistically significant (P<0.05). There were no significant differences in the incidences of acute labor and amniotic fluid pollution between the two groups (P>0.05). Conclusion Compared with Misoprostol induction of labor, the use of COOK double balloon for induction of labor in full-term pregnancy-induced hypertension can help reduce the incidence of complications after delivery, promote cervical ripening, shorten the time of labor, and increase the rate of vaginal delivery. It is worth promoting.
