集束化护理在无充气经腋入路全腔镜甲状腺切除术围手术期护理中的应用

摘要目的分析集束化护理在无充气经腋入路全腔镜甲状腺切除术围手术期护理中的应用效果。方法选取2020年5月至2021年5月于广东省农垦中心医院胸外科行无充气经腋入路全腔镜甲状腺切除术治疗的88 例患者作为研究对象，采用随机数字表法将其分为参照组与干预组，每组各44 例。参照组患者采取常规方法进行干预，干预组患者采取集束化护理进行干预。比较两组患者护理前后心理状态、并发症总发生率、护理舒适度、护理满意度评分、住院时间以及治疗费用。结果干预组患者护理后的焦虑自评量表（SAS）和抑郁自评量表（SDS）评分均高于参照组，差异有统计学意义（P＜0.05）。两组患者的并发症总发生率比较，差异无统计学意义（P＞0.05）。干预组患者的住院时间短于参照组，治疗费用少于参照组，差异有统计学意义（P＜0.05）。干预组患者的Kolcabal 评分及护理满意度评分均高于参照组，差异有统计学意义（P＜0.05）。结论对于采用无充气经腋入路全腔镜甲状腺切除术治疗的患者，在其围手术期护理干预中积极开展集束化护理更有利于改善其负面心理，不仅可以减少术后并发症的发生，缩短住院时间，而且还可以有效提高患者护理舒适度以及满意度，减少治疗费用。

Abstract：Objective To analyze the effect of cluster nursing in perioperative nursing of non inflatable transaxillary total endoscopic thyroidectomy.Methods A total of 88 patients who underwent total endoscopic thyroidectomy without inflatable through axillary approach in the Department of Thoracic Surgery,Guangdong Nongken Central Hospital from May 2020 to May 2021 were selected as the research subjects.They were divided into the reference group and the intervention group by random number table method,with 44 cases in each group.Patients in the reference group were treated with routine intervention,and patients in the intervention group were treated with cluster nursing.Psychological status,total incidence of complications,nursing comfort,nursing satisfaction score,length of stay and treatment cost were compared between the two groups before and after nursing.Results The scores of self-rating anxiety scale (SAS)and self-rating depression scale (SDS) in the intervention group were higher than those in the reference group,and the differences were statistically significant (P＜0.05).There was no statistically significant difference in the total incidence of complications between the two groups (P＞0.05).The duration of hospitalization in the intervention group was shorter than that in the reference group,and the treatment cost was less than that in the reference group,the differences were statistically significant (P＜0.05).Kolcabal score and nursing satisfaction score of patients in the intervention group were higher than those in the reference group,the differences were statistically significant (P＜0.05).Conclusion For patients treated with non inflatable transaxillary total endoscopic thyroidectomy,actively carrying out cluster nursing in their perioperative nursing intervention is more conducive to improve their negative psychology.It can not only reduce the occurrence of postoperative complications and shorten the length of hospital stay,but also effectively improve patients′nursing comfort and satisfaction and reduce treatment costs.
