针对性护理干预在预防宫颈癌术后尿潴留中的应用效果

摘要目的探讨针对性护理干预在预防宫颈癌术后尿潴留中的应用效果。方法选取2018年8月至2021年4月萍乡市妇幼保健院120 例宫颈癌根治术后出现尿潴留的患者，按照随机数字表法分为两组，各60 例。对照组行术后常规护理，观察组常规护理干预基础上行针对性护理干预，比较两组患者干预前后泌尿系统不适症状评分变化情况，比较两组患者干预前后尿流动力学指标中膀胱顺应性及膀胱最大逼尿肌压力，统计两组患者留置导尿时间、拔除导尿管后24 h 尿量和残尿量。结果干预后两组患者泌尿系统不适症状评分低于干预前，且观察组泌尿系统不适症状评分低于对照组，差异有统计学意义（P<0.05）。干预后观察组顺应性及膀胱最大逼尿肌压力均大于对照组，差异有统计学意义（P<0.05）。观察组留置导尿时间短于对照组，差异有统计学意义（P<0.05）。拔除导尿管后24 h 尿量大于对照组，残尿量少于对照组，差异有统计学意义（P<0.05）。结论针对性护理干预能有效改善宫颈癌术后尿潴留患者泌尿系统不适，改善膀胱生理功能，缩短术后留置导尿时间，促进排尿生理功能早期恢复。

Abstract：Objective To explore the clinical effect of targeted nursing intervention in preventing postoperative urinary retention of cervical cancer. Methods A total of 120 patients with urinary retention after radical hysterectomy for cervical cancer in Pingxiang Maternal and Child Health Hospital from August 2018 to April 2021 were divided into two groups according to random number table method, with 60 cases in each group. The control group was given routine nursing after operation, the observation group was given targeted nursing on the basis of routine nursing intervention.The changes of urinary discomfort symptom scores, bladder compliance and bladder maximum detrusor pressure in urodynamic indexes before and after intervention were compared between the two groups, and the time of indwelling catheter, urine volume and residual urine volume 24 h after catheter removal were counted between the two groups.Results After the intervention, the scores of urinary discomfort symptoms in the two groups were lower than those before the intervention, and the scores of urinary discomfort symptoms in the observation group were lower than those in the control group, the differences were statistically significant (P<0.05). After the intervention, the compliance and the maximum detrusor pressure in the bladder of the observation group were higher than those in the control group, the differences was statistically significant (P<0.05). The indwelling catheterization time in the observation group was shorter than that in the control group, the difference was statistically significant (P<0.05). The urine volume in 24 h after catheter removal was larger than that in the control group,the difference was statistically significant(P<0.05).Conclusion Targeted nursing can effectively improve urinary discomfort,improve bladder physiological function,shorten postoperative indwelling catheterization time and promote early recovery of urinary physiological function in patients with cervical cancer after operation.
