腹腔镜完全腹膜外疝修补术与腹腔镜经腹膜前疝修补术对单纯性腹股沟疝患者腹股沟疼痛、性功能和生活质量的影响比较

摘要

目的比较腹腔镜完全腹膜外疝修补术（TEP）和腹腔镜经腹膜前疝修补术（TAPP）对单纯性腹股沟疝患者腹股沟疼痛、性功能、生活质量的影响。方法选取2018年1月至2020年12月在无锡市锡山区中医医院普外科行腹腔镜腹股沟疝修补术治疗的88 例单纯性腹股沟疝患者作为研究对象，采用计算机随机数字表法分为TEP组和TAPP 组，各44 例。 TEP 组患者行TEP，TAPP 组患者行TAPP，比较两组患者术前和术后3、6 个月性功能和生活质量以及术前和术后1、6、24 h，1 周，1、3、6 个月腹股沟疼痛评分。结果两组患者术后性功能及生活质量均高于术前，差异有统计学意义（P<0.05）；TEP 组和TAPP 组性功能及生活质量评分比较，差异无统计学意义（P>0.05）。两组患者术后3、6 个月腹股沟疼痛评分低于术前；TEP 组术后6、24 h，1 周，1 个月腹股沟疼痛评分低于TAPP 组，差异有统计学意义（P<0.05）；TEP 组与TAPP 组术后1 h 及3、6 个月腹股沟疼痛评分比较，差异无统计学意义（P>0.05）。结论 TEP 和TAPP 均可改善患者腹股沟慢性疼痛、性功能和生活质量，TEP 组术后早期腹股沟疼痛评分低于TAPP 组，两组患者在长期疗效方面无差异。

Abstract：

Objective To compare the effect of totally extraperitoneal hernia repair (TEP) and transabdominal preperitoneal hernia repair (TAPP) on patients′ groin pain, sexual function and quality of life. Methods A total of 88 patients with inguinal hernia in Xishan District Hospital of Traditional Chinese Medicine from January 2018 to December 2020 were selected as research objects, they were divided into TEP group and TAPP group according to computer random number table method, with 44 cases in each group. Patients in the TEP group underwent TEP, and patients in the TAPP group underwent TAPP. The sexual function and quality of life were compared between the two groups before surgery, 3 and 6 months after surgery, as well as groin pain was compared between the two groups before surgery, at 1,6, 24 h, 1 week, 1, 3 and 6 months after surgery. Results The sexual function and quality of life postoperatively of two groups after surgery were higher than those before surgery, the differences were statistically significant (P<0.05). However, there were no significant differences in sexual function and quality of life between two groups before and after surgery (P>0.05). The groin pain of the two groups at 3 and 6 months after surgery were lower than those before surgery, the differences were statistically significant (P<0.05). The groin pain in the TEP group was lower than that in the TAPP group at 6, 24 h, 1 week and 1 month after surgery, the difference was statistically significant (P＜0.05). There were no significant differences in groin pain between two groups at 1 h, 3 and 6 months after surgery (P＞0.05). Conclusions TEP and TAPP can improve the sexual function, quality of life and groin pain. The early postoperative groin pain score in the TEP group is lower than that in the TAPP group, but TEP and TAPP repairs are comparable in terms of these long-term outcomes.
