丁卡因胶浆与石蜡油在经皮肾镜碎石取石术后留置导尿管中的应用效果比较

摘要目的比较丁卡因胶浆与石蜡油在男性泌尿生殖系统手术后留置导尿管中的应用效果。方法选取2017年1月至2021年8月中国人民解放军联勤保障部队第908 医院手术室收治的83 例全身麻醉下行经皮肾镜碎石取石术患者作为研究对象，采用随机数字表法将其分为丁卡因组（42 例）和石蜡油组（41 例）。丁卡因组采用丁卡因胶浆留置导尿管，石蜡油组采用石蜡油留置导尿管。比较两组患者注药时（T0）、拔除气管导管后5 min（T1）、10 min（T2）、30 min（T3）的血流动力学变化，术后苏醒期的躁动情况，术后8、24、48 h 的舒适度及尿道刺激反应情况。结果丁卡因组的心率（HR）、平均动脉压（MAP）呈下降趋势，血氧饱和度（SpO2）呈上升趋势；石蜡油组的HR、MAP变化无明显规律，SpO2 呈上升趋势。两组T0 时的HR、MAP 以及SpO2 水平比较，差异均无统计学意义（P＞0.05）；丁卡因组T1、T2、T3 时的HR 慢于石蜡油组，MAP 低于石蜡油组，差异有统计学意义（P＜0.05）；两组T1、T2、T3 时的SpO2 水平比较，差异无统计学意义（P＞0.05）。丁卡因组的术后苏醒躁动评分低于石蜡油组，差异有统计学意义（P＜0.05）；两组的GCQ 评分均呈逐渐上升趋势，且丁卡因组术后12、24、48 h 时的GCQ 评分均高于石蜡油组，差异有统计学意义（P＜0.05）。丁卡因组的尿道刺激反应发生率低于石蜡油组，差异有统计学意义（P＜0.05）。结论在男性泌尿生殖系统手术后留置导尿管中应用丁卡因胶浆有利于平稳患者的HR 与MAP，减轻患者术后苏醒躁动程度，提高患者术后舒适度，减少患者尿道刺激。

Abstract：Objective To compare the application effects of Tetracaine Mortar and Paraffin Oil in urinary catheter indwelling after male genitourinary system surgery.Methods A total of 83 patients with percutaneous nephrolithotomy under general anesthesia admitted to Operating Room of the 908th Hospital of China People's Liberation Army Joint Logistic Support Force from January 2017 to August 2021 were selected as the study subjects.According to the random number table method,the patients were divided into in the Tetracaine group(42 cases)and in the Paraffin Oil group(41 cases).The Tetracaine group was given Tetracaine Mortar for urinary catheter indwelling,and the Paraffin Oil group was given Paraffin Oil for urinary catheter indwelling.The hemodynamic changes at drug injection(T0)and at 5 min(T1),10 min(T2)and 30 min(T3)after removal of tracheal catheter,postoperative emergence agitation and comfort and urethral irritation response at 8,24 and 48 h after surgery were compared between the two groups of patients.Results The heart rate(HR)and mean arterial pressure(MAP)showed downward trends,while the blood oxygen saturation(SpO2)showed an upward trend in the Tetracaine group,but there were no obvious changes in HR and MAP and SpO2 showed an upward trend in the Paraffin Oil group.There were no statistical differences in the levels of HR,MAP and SpO2 between the two groups at T0(P＞0.05).The level of HR of the Tetracaine group at T1,T2 and T3 were slower than those of the paraffin oil group,and the level of MAP of the Tetracaine group at T1,T2 and T3 were lower than those of the Paraffin Oil group,the differences were statistically significant(P＜0.05).There were no statistical significances in the levels of SpO2 at T1,T2 and T3 between the two groups(P＞0.05).The postoperative emergence agitation score in the Tetracaine group was lower than that in the Paraffin Oil group,the difference was statistically significant(P＜0.05).The GCQ score of the two groups showed a gradually increasing trend,and the GCQ scores in the Tetracaine group at 12,24 and 48 h after surgery were higher than those in the Paraffin Oil group,the differences were statistically significant(P＜0.05).The incidence rate of urethral irritation response in the Tetracaine group was lower than that in the Paraffin Oil group,the difference was statistically significant(P＜0.05).Conclusion The application of Tetracaine Mortar in urinary catheter indwelling after male genitourinary system surgery is beneficial to stabilizing the heart rate and mean arterial pressure,relieving the postoperative emergence agitation,improving the postoperative comfort and reducing the urethral irritation of patients.
