替诺福韦酯联合拉米夫定治疗艾滋病合并乙型肝炎病毒感染的效果

摘要目的探讨替诺福韦酯联合拉米夫定治疗艾滋病合并乙型肝炎病毒感染的临床效果。方法选取2017年9月～2019年1月江西省赣州市第五人民医院收治的78例艾滋病合并乙型肝炎病毒感染患者作为研究对象，按照抽签法分为对照组和观察组，每组各39例。对照组单独采用拉米夫定治疗，观察组采用替诺福韦酯联合拉米夫定治疗。比较两组乙型肝炎病毒（HBV）DNA检测阳性率、HBV DNA含量、人类免疫缺陷病毒（HIV）RNA 转阴率、丙氨酸转氨酶（ALT）复常率及不良反应总发生率。结果治疗6个月、1年后，观察组HBV DNA检测阳性率及HBV DNA含量均低于对照组，差异有统计学意义（P<0.05）。治疗6个月、1年后，观察组HIV RNA 转阴率及ALT 复常率均高于对照组，差异有统计学意义（P<0.05）。两组不良反应总发生率比较，差异无统计学意义（P>0.05）。结论替诺福韦酯联合拉米夫定治疗艾滋病并乙型肝炎病毒感染具有有效性高、安全性好等优势，值得推广。

Abstract：Objective To explore the clinical effect of Tenofovir Dipivoxil combined with Lamivudine in the treatment of AIDS combined with hepatitis B virus infection.Methods The 78 patients of AIDS with hepatitis B virus infection admitted to the 5th People′s Hospital of Ganzhou from September 2017 to January 2019 were selected as the research subjects.They were divided into a control group and an observation group according to the method of drawing lots,39 cases in each group.The control group was treated with Lamivudine,the observation group was treated with Tenofovir Dipivoxil combined with Lamivudine.The positive rate and content of hepatitis B virus (HBV) DNA,RNA conversion rate of human immunodeficiency virus (HIV),recurrence rate of alanine aminotransferase (ALT) and the total incidence of adverse reactions rate were compared between the two groups..Results After 6 months and 1 year of treatment,the positive rate and content of HBV DNA in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).After 6 months and 1 year of treatment,the HIV RNA negative rate and ALT normalization rate in the observation group were higher than those in the control group,the differences were statistically significant (P<0.05).There was no significant difference in total incidence of adverse reactions between the two groups (P>0.05).Conclusion Tenofovir Dipivoxil combined with Lamivudine in the treatment of AIDS complicated with hepatitis B virus infection has the advantages of high efficacy and safety,which is worthy of promotion.
