靶向药物阿帕替尼辅助三维适形调强放疗治疗老年非小细胞肺癌患者的临床效果

摘要目的探讨靶向药物阿帕替尼药物辅助三维适形调强放疗治疗老年非小细胞肺癌患者的临床效果。方法选择2017年1月～2019年12月于盐城市第二人民医院就诊的80例老年非小细胞肺癌患者作为研究对象，采用随机数字表法将其分为观察组和对照组，每组各40例。对照组患者给予三维适形调强放疗，观察组患者在对照组的基础上给予阿帕替尼治疗。观察及对比两组患者的临床总有效率、治疗前后血清癌胚抗原（CEA）和血管内皮生长因子（VEGF）水平以及不良反应发生情况。结果治疗结束后1个月，观察组患者的临床总有效率为62.5%，高于对照组的45.0%，差异有统计学意义（P＜0.05)。治疗后，与治疗前相比，两组患者的血清CEA和VEGF水平均降低，且观察组患者的血清CEA和VEGF水平均低于对照组，差异有统计学意义（P＜0.05)。两组患者的主要不良反应为高血压、出血、胃肠道反应、血红蛋白减少、白细胞减少和肝肾功能不全；观察组患者的不良反应总发生率为17.5%，低于对照组的37.5%，差异有统计学意义（P＜0.05）。结论靶向药阿帕替尼药物辅助三维适形调强放疗治疗老年非小细胞肺癌患者的临床效果确切，血清CEA和VEGF水平得到显著改善，不良反应少，治疗效果显著，值得临床推广应用。

Abstract：Objective To investigate the clinical effect of targeted drug Apatinib assisted three-dimensional conformal intensity-modulated radiotherapy in the treatment of elderly patients with non-small cell lung cancer.Methods A total of 80 cases of elderly patients with non-small cell lung cancer who were treated in Yancheng Second People′s Hospital from January 2017 to December 2019 were selected as the subjects，and they were divided into the observation group and the control group using the random number table method，40 cases in each group.The control group was given three-dimensional conformal intensity-modulated radiotherapy，the observation group was given Apatinib on the basis of the control group，the total clinical effective rate，serum carcinoembryonic antigen (CEA) and vascular endothelial growth factor (VEGF) levels before and after treatment and incidence of adverse reactions of the two groups were compared.Results One month after the end of the treatment，the total clinical effective rate of the observation group was 62.5%，which was higher than that of the control group (45.0%)，the difference was statistically significant (P＜0.05).After treatment，compared with before treatment，the serum CEA and VEGF levels of the two groups decreased，and the serum CEA and VEGF levels of the observation group were lower than those of the control group，the differences were statistically significant (P＜0.05).The main adverse reactions of the two groups were hypertension，hemorrhage，gastrointestinal reaction，hemoglobin reduction，leucopenia and liver and kidney dysfunction.The total incidence rate of adverse reactions in the observation group was 17.5%，which was lower than that in the control group (37.5%)，the difference was statistically significant (P＜0.05).Conclusion The effect of Apatinib in the combination with three-dimensional conformal intensity-modulated radiotherapy in the treatment of elderly patients with non-small cell lung cancer is definite，the serum CEA and VEGF levels are significantly improved，the adverse reactions are few，and the therapeutic effect is significant，which is worthy of clinical application.
