人文关怀护理干预对喉癌手术患者自护能力、心理状态及生活质量的影响

摘要目的探讨人文关怀护理干预在喉癌手术患者中的应用效果。方法选取2015年11月～2019年11月江西省肿瘤医院收治的94例喉癌患者作为研究对象，采用随机数字表法将其分为对照组和观察组，每组各47例。对照组实施常规护理，观察组采取人文关怀护理，比较两组干预前和干预7 d后自我护理能力测定量表（ESCA）、心理状态[焦虑自评量表（SAS）、抑郁自评量表（SDS）]及生活质量核心量表（QLQ-C30）评分。结果两组干预前ESCA评分、SAS评分、SDS评分、QLQ-C30 评分比较，差异无统计学意义（P＞0.05）。观察组干预后的ESCA 自护概念评分、健康知识水平评分、自护责任感评分及自护能力评分均高于对照组，差异有统计学意义（P＜0.05）。观察组干预后的SAS评分与SDS评分均低于对照组，差异有统计学意义（P＜0.05）。观察组干预后QLQ-C30 情绪评分、躯体功能评分、认知能力评分、社会功能评分及家庭角色评分均高于对照组，差异有统计学意义（P＜0.05）。结论人文关怀护理干预能够提高喉癌手术患者自护能力，改善患者负面情绪，提升生活质量。

Abstract：Objective To explore the effect of humanistic care as a nursing intervention in patients undergoing laryngeal cancer surgery.Methods A total of 94 patients with laryngeal cancer were admitted to our hospital from November 2015 to November 2019 were selected as the research subjects.They were divided into the control group and the observation group according to the random number table,47 cases in each group.The control group received routine nursing,while the observation group received humanistic care.The scores of exercise of self-care agency (ESCA) scale,mental state including self-rating anxiety scale (SAS),self-rating depression scale (SDS),and core quality of life(QLQ-C30) scale were compared between the two groups before intervention and 7 days after intervention.Results There were no statistically significant differences in ESCA score,SAS score,SDS score,or QLQ-C30 score between the two groups before intervention (P＞0.05).In the observation group,the scores of ESCA self-care concept,health knowledge,self-care responsibility,and self-care ability after the intervention all higher than those of the control group,the differences were statistically significance (P＜0.05).The SAS and SDS scores of the observation group after intervention were lower than those of the control group,the differences were statistically significance (P＜0.05).The scores of QLQC30 emotional function,physical function,cognitive ability,social function,family role function in the observation group after intervention were all higher than those of the control group,the differences were statistically significance(P＜0.05).Conclusion Humanistic care as a nursing intervention can enhance the self-care ability of patients undergoing laryngeal cancer surgery,improve the negative emotions,and ameliorate the QOL.
