品管圈在降低辅助用药占比管理中的应用研究

摘要目的探讨品管圈质量管理工具在辅助用药管理中的应用，以期降低其占比，促进临床合理用药。方法组建品管圈，收集赣州市妇幼保健院2019年1～4月辅助用药数据，选定“降低辅助用药占比”为活动主题，利用柏拉图、鱼骨图、80/20 法则分析辅助用药使用不合理的原因，于2019年6～12月制订并实施人员教育、信息化手段干预、行政干预、事后处方点评干预等多种对策，比较品管圈活动前后辅助用不合理使用情况、收入占比及圈员成长值评测等指标，评价活动效果。结果辅助用药主要不合理使用类别累计占比从活动前的78.95%降至65.81%；辅助用药收入占比从活动前的14.86%降至活动后的6.84%，低于目标值7.30%，目标达成率为106.00%，改善率为53.93%；圈员活动成长值显著提高，以解决问题能力和学习积极性改善最明显。结论运用品管圈质量管理工具可有效降低辅助用药占比，促进其合理使用，有利于医疗质量持续改进，值得推广。

Abstract： Objective To investigate the application of quality control circle in the management of assistant medication,aiming to reduce its drug cost index and improve the rational drug use in the clinic. Methods The data of assistant medication from January to April 2019 in Maternity and Child Care Hospital of Ganzhou City were analyzed by plato and fishbone diagram and 80/20 rule for finding the causes of improper use of assistant medication around the topic"reducing the drug cost index of assistant medication" after structuring quality control circle. Education, informationbased means, administrative intervention and post-prescription comment were formulated and implemented in quality control circle from June to December 2019. The utilization and income of assistant medication and the progressive rate of circle members were compared before and after the quality control circle. Results The utilization and income of assistant medication decreased from 78.95% before quality control circle to 65.81% after quality control circle, and the income of assistant medication decreased from 14.86% before quality control circle to 6.84% after quality control circle, below the target value (7.30%), while the rate of goal achievement was 106.00% and the rate of improvement was 53.93%. The progressive rate of circle members increased positively significantly, while their ability to solve problems and enthusiasm for learning were increased most obviously. Conclusion The quality control circle is worthy of promotion for reducing the drug cost index of assistant medication, improving the rational drug use in the clinic and improving the medical quality continuously.
