Influencing factors analysis of macroangiopathy in patients with hypertension and type 2 diabetes mellitus and its countermeasures

Abstract:Objective To investigate the influencing factors of macroangiopathy in patients with hypertension and type 2 diabetes mellitus and its countermeasures. Methods The clinical data of 200 patients with hypertension and type 2 diabetes mellitus who were treated in the Third Hospital of Nanchang from November 2018 to November 2019 were retrospectively analyzed, and they were divided into lesion group (n=120) and non-lesion group (n=80) according to the abnormal changes of carotid artery intima. The general data and biochemical indexes of two groups were collected, and the influencing factors of macroangiopathy in patients with hypertension and type 2 diabetes mellitus were analyzed.The lesion group was divided into two groups according to different intervention methods, with 60 cases in each group.Group A was given routine treatment, and group B added lifestyle intervention. The situations of patients before intervention and after intervention for 12 weeks were compared. Results The fasting insulin (FINS), intima-media thickness (IMT), homeostasis model assessment of insulin resistance (HOMA-IR) and homeostasis model assessment-β cell function (HOMA-βF) in lesion group were higher than those in non-lesion group, with statistically significant differences (P<0.05). There were no statistical differences in the body mass index (BMI), fasting plasma glucose (FPG)and glycated hemoglobin (HbA1c) between the lesion group and the non-lesion group (P>0.05). Multivariate Logistic regression analysis found that FINS≥55.50 pmol/L, IMT≥1.10 mm, HOMA-IR≥3.25%, and HOMA-βF≥3.40%were risk factors for macroangiopathy in patients with hypertension and type 2 diabetes mellitus (OR>1, P<0.05). After intervention, the levels of FINS and HOMA-IR in group B were lower than those in group A, and the differences were statistically significant(P<0.05).Conclusion FINS≥55.50 pmol/L, IMT≥1.10 mm, HOMA-IR≥3.25%, and HOMA-βF≥3.40% are risk factors of macroangiopathy in patients with hypertension and type 2 diabetes mellitus. Early treatment and effective lifestyle intervention can improve insulin resistance, and delay the development of macroangiopathy.
