戈舍瑞林联合腹腔镜术对子宫内膜异位症患者性激素及血管相关指标水平的影响

摘要目的观察戈舍瑞林联合腹腔镜术对子宫内膜异位症（EMT）患者性激素及血管相关指标水平的影响。方法选择2016年11月～2018年10月我院治疗的92 例EMT 患者，按随机数字表法分为两组，每组各46 例。对照组接受腹腔镜术治疗，观察组接受戈舍瑞林、腹腔镜术治疗。比较两组性激素[卵泡刺激素（FSH）、雌二醇（E2）、促黄体生成素（LH）]、血管相关指标[血管生成素-2（Ang-2）、血管生长因子（VEGF）]、不良反应及复发率。结果治疗前两组FSH、E2、LH、Ang-2、VEGF 水平比较，差异无统计学意义（P＞0.05）；治疗后观察组FSH[（3.88±0.97）mmol/L]、E2[（12.45±4.18）mmol/L]、LH[（0.58±0.22）mmol/L]、Ang-2[（2.14±0.85）mg/L]、VEGF[（1.12±0.41）mg/L]，低于对照组[FSH（4.90±1.15）mmol/L，（23.14±6.79）mmol/L，（1.48±0.54）mmol/L，（3.21±1.10）mg/L，（1.81±0.65）mg/L]，差异均有统计学意义（P＜0.05）；观察组的复发率为4.35%，低于对照组的19.57%，差异有统计学意义（P＜0.05）；两组的不良反应总发生率比较，差异无统计学意义（P＞0.05）。结论戈舍瑞林、腹腔镜术联合治疗能够降低EMT 复发率，改善性激素水平及血管相关指标，且不良反应较少。

Abstract：Objective To observe the effect of Goserelin combined with laparoscopic surgery on the levels of sex hormones and vascular-related indices in the patients with endometriosis (EMT). Methods A total of 92 patients with EMT who were treated in our hospital from November 2016 to October 2018 were selected and divided into two groups according to the random number table method, with 46 patients in each group. The control group was given laparoscopic surgery, and the observation group was given Goserelin combined with laparoscopic surgery. The sex hormones (follicle stimulating hormone [FSH], estradiol [E2], luteinizing hormone [LH]), vascular-related indices (angiogenin-2 [Ang-2],vascular growth factor [VEGF]), adverse reactions and recurrence rate were compared between the two groups. Results The differences of the levels of FSH, E2, LH, Ang-2 and VEGF in the two groups before treatment were not statistically significant (P＞0.05); after treatment, FSH ([3.88±0.97]mmol/L), E2 ([12.45±4.18]mmol/L), LH ([0.58±0.22]mmol/L),Ang-2 ([2.14±0.85]mg/L) and VEGF ([1.12±0.41]mg/L) in the observation group were respectively lower than those in the control group ([4.90±1.15]mmol/L, [23.14±6.79]mmol/L, [1.48±0.54]mmol/L, [3.21±1.10]mg/L, [1.81±0.65]mg/L),and the differences were statistically significant (P＜0.05); the recurrence rate in the observation group was 4.35%, lower than 19.57% in the control group, and the difference was statistically significant (P＜0.05); the difference in the total incidence rate of adverse reactions between the two groups was not statistically significant (P ＞0.05). Conclusion Goserelin combined with laparoscopic surgery in the treatment of EMT can reduce the recurrence rate, improve patients' sex hormone levels and vascular-related indices,and have fewer adverse reactions.
