Abstract:Objective To study effect of positive end expiratory pressure(PEEP)combined with prone ventilation on oxygenation and circulation in patients with acute respiratory distress syndrome(ARDS).Methods From June 2013 to September 2014,a total of 80 patients with ARDS in our hospital were taken as research objects and divided into control group(40 cases)and observation group(40 cases)by random number table method. Both groups were treated with PEEP,patients in the observation group accepted additional prone ventilation.Oxygenation indexes, intensive care unit (ICU)stay time, mechanical ventilation time, mean arterial pressure(MAP)and heart rate(HR)of two groups were compared and analyzed, and 28 d mortality rates and complication rates of two groups were recorded. Results Oxygenation index of the observation group was 162.14±2.87,which was significantly higher than 135.76±1.89 of the control group (P<0.05);ICU stay time and mechanical ventilation time of the observation group was significantly shorter than that of the control group(P<0.05).There were no significant differences in each group or between two groups in MAP and HR before and after treatment(P>0.05).28 d mortality rate of the observation group was 2.50%,which was significantly lower than 17.50%of the control group(P<0.05);complication rates of two groups was respectively 7.50%and 5.00%,and there was no significant difference between two groups(P>0.05).Conclusion PEEP combined with prone ventilation has the advantage of less effect on blood circulation and no obvious increase of serious complications, can effectively improve oxygenation and prognosis for ARDS patients. Thus,it is worthy of clinical application and promotion
